What is Hospice Care?
Hospice focuses on caring, not curing. Hospice is a care model that delivers quality comfort supportive care for any individual with a life-limiting illness. Hospice is a wholistic approach which includes medical, emotional, physical, and spiritual management thru a team of experts. The interdisciplinary team includes; hospice medical director, RN case manager, licensed vocational nurse, home health aide, social worker, chaplain and volunteers.
Services include:
- Pain management
- Symptom management
- Support with bathing & grooming
- Assist families with emotional support
- Educate families on caregiving for loved one
- Provide grief counseling
- Placement
Who is Eligible for Hospice Care?
Early identification of hospice eligible patients helps them receive the comfort care they need and deserve. To be eligible for hospice, a patient must have a terminal illness with a prognosis of 6 months or less. A licensed healthcare professional must certify that the patient qualifies for hospice care. A Aasta Hospice admissions nurse is available 24/7 days a week to assess eligibility of a patient in need of care.
